What is Duac gel for acne and how does it work?

Duac gel for acne is a prescription-only topical medicine (Stiefel/GSK) used for acne vulgaris in people aged 12 and over. It combines clindamycin 1% with benzoyl peroxide (BPO) 5%, authorised in Spain by the medicines regulator (AEMPS). In the UK, equivalent clindamycin/BPO combinations are prescription-only and regulated by the MHRA.

Clindamycin inhibits protein synthesis in Cutibacterium acnes by binding to the 50S ribosomal subunit. Used on its own, it can drive antibiotic resistance relatively quickly. BPO adds a bactericidal effect via free radicals (a physico-chemical mechanism that does not create bacterial resistance) plus a mild comedolytic action. The combination performs better than either ingredient alone; importantly, BPO actively reduces selection of clindamycin-resistant strains.

Duac gel for acne price: what it costs in 2026

It is sold as a 25 g tube. A typical price in 2026 is around €17–€22. In Spain it is not funded by the public health system for most indications, so you pay the full retail price. A complete monthly regimen (Duac + cleanser + SPF) usually comes to roughly €35–€60 per month.

Who Duac gel for acne is for (and when it’s not the best option)

It’s indicated for mild-to-moderate inflammatory acne vulgaris: papules and pustules on the face, neck, back or chest, without extensive nodules or cysts, particularly when you haven’t responded well enough to BPO alone or azelaic acid. It can also fit adult hormonal acne when inflammation is the dominant feature.

If your acne is mainly comedonal (lots of blackheads/whiteheads), retinoids such as adapalene or tretinoin are often more suitable. For severe nodulocystic acne, oral isotretinoin is the standard option under specialist care. Contraindications: hypersensitivity to clindamycin, lincomycin or BPO; history of antibiotic-associated colitis; pregnancy (category C); under 12 years old. Do not apply to eyes, mucous membranes or broken skin.

How to use Duac gel for acne properly: step-by-step protocol

The standard regimen per the product information: apply once daily at night, on clean, dry skin, as a thin layer over affected areas.

You’ll usually see improvement from weeks 4–6; the full effect tends to be between weeks 8 and 12. Impatience is the most common mistake I see. Other frequent errors: applying on damp skin, using too much product, skipping SPF, or combining with retinoids/acids without medical guidance. Storage: keep refrigerated (2–8 ºC) before opening; once opened, use within 2 months at room temperature.

Side effects of Duac gel for acne and how to minimise them

Around 10–20% of people notice dryness, redness, itching or peeling during the first few weeks—mild local effects that usually settle as your skin adapts. For sensitive skin, starting every other night for the first 7–10 days is common clinical practice—and what I recommend.

Systemic side effects are extremely rare. If you develop severe diarrhoea or abdominal pain, stop and seek medical advice urgently (theoretical risk of pseudomembranous colitis). BPO increases sun sensitivity: without SPF, your risk of post-inflammatory hyperpigmentation goes up. Without a prescription, there isn’t a true like-for-like equivalent in efficacy. But there are actives with solid evidence: BPO alone at 2.5–5% for mild acne; azelaic acid 10% with antibacterial activity against C. acnes, plus an added benefit for post-inflammatory hyperpigmentation; niacinamide 4–10% with a moderate anti-inflammatory effect.
