A healthy vaginal flora is your first barrier against urinary tract infections, thrush and bacterial vaginosis. When it becomes unbalanced (after antibiotics, stress, hormonal changes or a public pool), you get symptoms most women recognise straight away: itching, different discharge, unpleasant odour, stinging when you pass urine.

In my practice I have spent years recommending probiotics for women at the pharmacy counter, and the question I hear most is always the same: “are they all the same?”. The honest answer is no. The formula changes, the strains change, and when you take them matters more than most people think.

This ranking includes the probiotics that in my experience really work in adult women: products with strains backed by solid studies (especially Lactobacillus rhamnosus, L. crispatus and L. reuteri), with proven vaginal colonisation and good tolerability in the women who come to the pharmacy.

1. Profaes4 Mujer — the balanced option I recommend most often

3. Cysticlean Prob — my choice if you have recurrent urinary infections

Cysticlean Prob is not just a probiotic: it combines Lactobacillus with PAC-A (proanthocyanidins from cranberry at 240 mg). This combination is among the most studied in women with recurrent cystitis (three or more episodes per year). The probiotic component helps repopulate vaginal flora; cranberry reduces adhesion of E. coli to bladder walls.

At the counter I recommend three-month courses as prevention (not as treatment for an acute cystitis — that needs antibiotics prescribed by your doctor). If you are currently having symptoms, see your GP or urgent care; if you have had several infections this year, this kind of product can make a real difference between episodes.

4. Alflorex Dual Action — the most clinically studied option

Alflorex Dual Action contains the patented strain Bifidobacterium longum 35624, one of the best documented in scientific literature (over 60 publications). Although its main indication is irritable bowel syndrome, its effect on overall microbiota balance can indirectly benefit intimate flora, especially in women whose digestive problems go hand in hand with vaginal discomfort.

It is more expensive than the others — I am aware of that. But if you have tried several probiotics without noticing much change, this one often stands out. Take one capsule daily for at least eight weeks before judging results.

5. Lactoflora Protector Íntimo — the pharmacy classic (temporarily out of stock)

Lactoflora Protector Íntimo remains one of the best-known references in this segment. It combines two specific strains for vaginal flora (L. rhamnosus and L. plantarum) and is taken as one capsule per day for 20 days. The advantages: simple schedule, good efficacy profile and a brand many women recognise and buy without hesitation.

It works very well for women who have completed a more intensive course (Profaes4 Mujer, DonnaPlus or Cysticlean Prob) and want to move into a long-term maintenance phase without stopping completely.

  • Certain oral probiotics containing lactobacilli can colonise the vagina via the gut–vaginal axis and support urogenital health (PMID: 32003642).
  • Cranberry proanthocyanidins combined with probiotics may reduce recurrence of urinary tract infections in some women (PMID: 31204476).

My recommendation according to your situation

After antibiotics or recurrent thrush

For maintenance during menopause

You can combine it with hyaluronic acid vaginal creams if dryness is an issue; local oestrogen may also be considered under medical supervision.

If you are pregnant or breastfeeding

This is where we need to be cautious: do NOT start anything new without checking with your midwife or obstetrician first. You usually notice a difference between the second and third week of daily use. Deeper changes (fewer recurrent episodes, more stable discharge) typically need 8 to 12 weeks of continuous treatment. If you have been taking it for a month and notice absolutely nothing, the strain is probably not suitable for you.

Can you take intimate probiotics during pregnancy?

Most intimate probiotics are safe in pregnancy, but always with your gynaecologist’s approval. The strains Lactobacillus rhamnosus and L. reuteri are the best studied in pregnancy and breastfeeding. Do not make decisions on your own during these stages.

Which works better: oral intimate probiotic or direct vaginal probiotic?

Do intimate probiotics help with unpleasant intimate odour?

They help IF the origin of the odour is an imbalance in the vaginal flora (subclinical bacterial vaginosis, excess Gardnerella). If the smell is very strong, persistent or comes with a change in discharge, first rule out bacterial vaginosis with your gynaecologist. An intimate probiotic is not a deodorant.

Can I take intimate probiotics at the same time as an antibiotic?

Yes, and in fact that is what I recommend. The rule: leave at least 2–3 hours between taking the probiotic and taking the antibiotic (so that the antibiotic does not kill the lactobacilli before they act). And continue the probiotic for at least 30 days after finishing the antibiotic course.
