Magnesium L-threonate is the only magnesium salt with documented ability to cross the blood–brain barrier in a clinically meaningful way, which makes it the salt of choice for cognitive indications (memory, concentration, executive function). Developed at MIT and marketed as Magtein®, it has attracted clinical interest since the trial by Slutsky et al. (2010). In this guide I explain what it is, what it is used for, what dose to take and what to do when you cannot find it in UK pharmacies.

What is magnesium L-threonate

It is a salt formed by magnesium bound to L-threonic acid (a vitamin C metabolite). The standardised form is Magtein®, developed by MIT researchers in the early 2010s with a specific goal: to create a salt that could cross the blood–brain barrier (BBB) and raise brain magnesium levels, not just plasma levels.

Why crossing the blood–brain barrier matters

The BBB is a filter that protects the brain from large or highly charged molecules. Most magnesium salts raise plasma magnesium but barely change brain levels: the Mg²⁺ ion crosses the BBB with difficulty. L-threonate, in contrast, does cross in a significant way. That property justifies its preferential use for cognitive indications.

The MIT study that shifted clinical interest

Slutsky et al. (Neuron, 2010) showed that L-threonate raises brain magnesium, increases synaptic density in the hippocampus and improves memory. Later human studies show improvements in executive function and attention in adults with mild cognitive complaints. The evidence base is less robust than for magnesium bisglycinate but specifically focused on cognition.

Bioavailability and particular features of L-threonate

Intestinal bioavailability is good (similar to citrate or bisglycinate). What sets it apart is not how much is absorbed but where it goes: L-threonate has the best documented brain penetration of any magnesium form. The elemental magnesium distributes preferentially to the central nervous system rather than remaining mainly in plasma, bone and muscle.

What is magnesium L-threonate used for

These are the situations where it makes sense to choose L-threonate over other magnesium supplements.

Support in mild cognitive impairment

Older adults with frequent forgetfulness, difficulty concentrating or mental slowing (without diagnosed dementia). Early studies suggest improvement in executive function after 12 weeks. It is not a curative treatment and does not replace neurological assessment if there is suspicion of underlying disease.

Improving memory and concentration

People doing intensive intellectual work with high cognitive demands, students under heavy academic load, professionals with demanding multitasking. A continuous course of 8–12 weeks allows you to assess effect. The improvement is not dramatic but can be noticeable in people with room for optimisation.

Support in anxiety with cognitive overdrive

Anxiety with racing thoughts, mental rumination and difficulty switching off. The effect on brain NMDA receptors adds central modulation to the peripheral effect you see with bisglycinate. This indication is less established but there is emerging literature.

Deep sleep (quality rather than sleep onset)

There are data suggesting improvement in sleep architecture (more deep sleep). For getting off to sleep, bisglycinate remains my first choice. For depth and sustained quality of sleep, L-threonate can add value.

Correct dose of magnesium L-threonate

This follows the specific regimen used with Magtein®.

Typical dose: 1,000–2,000 mg of L-threonate per day (144–288 mg of elemental magnesium).

Classic regimen: 3 capsules a day divided into 2 doses (morning and night).

Therapeutic minimum: 12 continuous weeks at full dose to assess real cognitive effect.

Time of day: one dose in the morning and one at night. The night-time dose helps sleep architecture.

Why you may not find pure L-threonate in UK pharmacies

I prefer to be transparent as a pharmacist: L-threonate is a premium salt and its presence in community pharmacies has been limited up to 2026. Reasons include active ingredient cost (patented Magtein®), a relatively niche cognitive indication and the fact that the evidence base is still relatively new. When you do see it, it is often via imported food supplements rather than standard OTC lines.

These are the two magnesium forms with particular affinity for the nervous system, but their profiles differ.

L-threonate has an edge for: cognitive function (memory, concentration, sustained mental performance), depth of sleep and indications clearly centred on the CNS.

Bisglycinate has an edge for: sleep onset and general rest, generalised anxiety, muscle cramps, digestive tolerance, availability and price.

When NOT to take L-threonate without supervision

Pregnancy and breastfeeding (insufficient literature). Severe renal impairment. Concomitant use with bisphosphonates or antibiotics (separate by at least 2 hours). Active psychiatric disorders under treatment (discuss with your psychiatrist). Under 18 years of age.

When to expect results with L-threonate

Sleep quality and depth: around 2–4 weeks. Concentration and short-term memory: around 4–8 weeks. Measurable cognitive effect on formal tests: at least 12 weeks. This salt works slowly and cumulatively — keep going for at least 3 months before making a serious judgement.

Common mistakes with L-threonate

Mistake 1: buying "threonate" without checking for Magtein®. Not all products labelled as threonate use the standardised form tested in trials. Looking for Magtein® on the label is a useful quality marker.

Mistake 2: expecting results within 1–2 weeks. It takes longer to show cognitive effects than bisglycinate does for sleep support. Allow at least 12 weeks.

Mistake 3: paying premium prices without a clear rationale. If cognition is not clearly your main goal, the extra cost usually does not pay off. Correctly dosed bisglycinate covers most objectives.

Frequently asked questions about L-threonate

Does L-threonate really cross the blood–brain barrier? Yes, according to Slutsky et al. (2010) and later studies. It is the only magnesium salt with this property documented to a significant degree.

Is it the best magnesium for memory? It is the best magnesium specifically for cognitive indications if that is your priority. For sleep support, cramps or muscle function it would not be my first choice.

Are Magtein® and L-threonate the same thing? Magtein® is the patented, standardised commercial form of magnesium L-threonate. Most clinical studies have used Magtein®. Generic versions may not have identical standardisation.

Can it be taken with other cognitive supplements? In general yes. It can be combined with omega‑3 fatty acids, B‑complex vitamins or phosphatidylserine. Avoid taking it at exactly the same time as high doses of caffeine. If you are on neurological medication, discuss combinations with your specialist.

L-threonate is currently the premium magnesium salt for cognitive indications, with solid early evidence and a specific mechanism of action. If your priority is memory, concentration or sustained cognitive performance, it is worth considering where available.

I also want to be realistic: availability in community pharmacies remains limited and cost is high compared with other forms.
