Which vaginal pessary is good for intimate cleansing?

For intimate cleansing the most used in Spanish pharmacy are Cumlaude Lab CLX (with chlorhexidine), Melagyn Pessaries (melaleuca and propolis) and Seidigyn. The choice depends on the indication: CLX is the most used option in post-menstrual cleansing and before gynaecological reviews; Melagyn provides milder antimicrobial action; Seidigyn combines cleansing and discomfort relief. Usual regimen is 1 pessary daily for 5-10 days.

What pessaries are there for vaginal dryness?

For vaginal dryness the most recommended are Mucogyne Pessaries (mucopolysaccharides), Cumlaude Lab Lubripiú (hyaluronic acid) and Seidigyn (polycarbophil). Mainly indicated for dryness associated with menopause, postpartum, breastfeeding or use of hormonal contraceptives. Start regimen is usually 1 pessary every 2-3 days for 2-3 weeks, then maintenance. If dryness is severe, it's advisable to combine them with daily hydrating gel.

When to use prebiotic pessaries?

Prebiotic pessaries (Cumlaude Lab Prebiotic, Gineseda) are indicated for restoring vaginal flora in specific situations: after oral antibiotic treatment (which also wipes out vaginal flora), after a thrush episode, in women with recurrent thrush as preventive regimen, or in any flora imbalance. Typical regimen: 1 pessary daily for 7-14 days.

How do you insert a vaginal pessary?

With clean hands, lie on your back with knees bent. Insert the pessary into the vagina as deep as you can with your finger, or use the applicator if the product includes one. Stay lying down for at least 10-15 minutes so it dissolves without coming out. Night-time application before sleep is the most convenient. The next day a small leak of dissolved excipient may appear: it's normal and doesn't mean the treatment has failed.

Can vaginal pessaries be used during menstruation?

It depends on the pessary. Cleansing pessaries can be used at the end of the period, when flow is scarce, to accelerate pH restoration. Hydrating or prebiotic pessaries aren't recommended during menstruation because blood washes away the product and reduces its efficacy. Wait 1-2 days after the bleeding ends to start the regimen. Menstruation itself isn't a contraindication, but it does reduce efficacy.

Do vaginal pessaries need a medical prescription?

In breastfeeding use is more flexible, but it's still recommended to consult with the midwife first.

Do vaginal pessaries work for thrush?

For active thrush the treatment of choice is prescribed vaginal antifungals (clotrimazole, fenticonazole). Cosmetic or prebiotic pessaries are adjuvants: they help restore flora once the infection is treated and to prevent recurrences. If you have thrush symptoms (itching, white lumpy discharge, burning), consult first with your gynaecologist or pharmacist for the right treatment.
